CHRONIC CARE
Generally, some 70%-80% of employers’ healthcare dollars are spent on covered individuals who have one or more chronic health conditions. To reduce these costs, HRM has developed an intensive, face to face approach to managing the risks of your chronically ill population.
Traditional disease management programs can be effective in reducing medical costs for patients with serious health conditions. However, many patients fail to become active participants or fail to improve their health with just telephonic support. Therefore, face to face intervention with a specially trained clinician, who will develop a trusted relationship with the patient, is needed. This approach, coupled with the use of appropriate consumer incentives by the patients’ employer, has proven to have a significant ROI.
Partnering with healthcare providers, including hospital systems or physician groups, HRM will identify clients’ patients who have chronic conditions that are manageable. HRM provides training and guidance to clinicians employed by these partners to ensure that patients referred to them from multiple employers within the same geographic region reach their clinical goals.
HRM focuses on a number of different manageable health conditions. Conditions such as Diabetes, Cardiac, Asthma, and Obesity are very manageable when the patient becomes compliant with state of the art treatment protocols.
By providing clinicians and facilities specifically dedicated to the on-going management of the chronically ill, HRM with its’ provider partners are able to deliver a focused level of care coordination for individuals with these conditions to reduce their health risk factors, lower their medical claims costs, improve productivity, and quality of life for affected individuals.
